We MUST have a VOIDED CHECK and a SIGNED Enrollment Form to start your new unlimited membership! For a credit card payment (debit cards not accepted, use voided check), please fill out the form below. Upon receipt of a signed Enrollment Form and voided check, or credit card information, your membership will start and $49 will be debited from your account each month

$49 a month you may have your choice of one of these services monthly: Facial (cleansing, hydrating, anti-aging, and purifying), back facial, full body wrap treatment, 2 partial body wraps, paraffin hand and foot treatment, revitalizing body scrub, detox foot bath and brazillian wax.
